Clinical Nursing Services:

Coordination of Patient Care including Enhanced Primary Care:

45 - 49 year old Health Assessments
75+ Health Assessments
Team Care Arrangements
Diabetes 12 month Cycle of Care
Asthma Cycle of Care
GP Management Plans
Liaising with other Allied Health care providers and community services

Maintaining the Clinical Environment:

Equipment maintenance including autoclaving (sterilisation)
Stock control, rotation and ordering
Cold chain monitoring

Administrative Duties:

Recall and reminders
Assistance with accreditation
Clinical and data base management

Health Promotion:

Patient Education
Health Promotion Programs e.g. Quit
Health Counselling

Anticipated Sessional Requirements ( minimum of 2 full days equivalent)

Certain criteria need to be met to participate in the Practice Nurse Initiative to support the intended outcomes of the project. Where some criteria are not met the Divisions of General Practice can provide support in meeting the relevant criteria.
