About the initiative
GP Plus Practice Nurse Initiative
To date the GP plus practice nurse Initiative has been implemented across 5 Divisions in the Central Northern region of Adelaide. It has supported the recruitment of more than 155 nurses in over 145 practices.
This phase of the GP plus practice Nurse Initiative will build on these gains by: Supporting nurses to move into "advanced roles" in General Practice. Supporting practices with a "whole of practice" approach to collaborate and create comprehensive systems to improve patient care. Targeting Chronic Disease and "at risk of hospitalisation" patients.
GP Plus Practice Nurses work collaboratively with GP's, acting as a catalyst for change within General Practice. They are involved in promoting good health and preventative health care by increasing the focus on chronic disease management. In addition they conduct health screening checks on population groups and provide education to individuals and the community.
Nurses involved in the Care coordination aspect of the project, are involved in coordinating comprehensive general practice care of patients "at risk of hospitalisation" Nurses will case find patients for initial needs assessment, comprehensive assessment and care planning. They act as a key worker who coordinates, communicates, navigates and advocates for the patients at risk of hospitalisation.

The Adelaide Health Service works in partnership with the Adelaide North East Division of General Practice as the lead agency for this initiative.
ANEDGP is working collaboratively with other metropolitan Divisions of General Practice to achieve the project aims in the following areas:
North - Adelaide Northern Division of General Practice
Hills - Adelaide Hills Division of General Practice
East and city - GP partners Adelaide
West - Healthfirst Network
In addition General Practice Network South is conducting a similar program in partnership with the Southern Adelaide Health Service and enquiries relating to that area should be directed to the General Practice Network South.
